Financial Policy

As a condition of treatment by this office, payment is due at the time service is provided; any financial arrangements must be made in advance. The practice depends upon reimbursement from patients for the costs incurred in their care. Before treatment is performed, we will discuss treatment and financial options.  This will allow you to fully understand your dental treatment, what to anticipate in fees and allow you time to make the necessary financial arrangements.

All emergency dental services, or any dental services performed without previous financial agreements, must be paid for at the time services are performed. Our office accepts cash, checks, MasterCard, Visa, Discover, American Express and Care Credit.

Insurance benefits are determined by your employer, not your dentist.  Your insurance policy is a contract between you and your insurance company.  Your insurance coverage and benefits are your responsibility.  Insurance is not a guarantee of payment; it often does not cover all the costs involved in treatment. As a courtesy, we will be happy to file your claim for you if you present your dental insurance card and all required employer information.  You will be expected to pay for services rendered if the office is unable to verify your insurance information before treatment. All charges you incur are your responsibility, regardless of your insurance coverage. We must emphasize that as your dental care provider, our relationship is with you, our patient, not with your insurance company.

Any deductible or estimated co-insurance amount will be due at the time of treatment. If payment for services already rendered has not been paid in full within 60 days, either by you or your insurance company, the remaining balance for your treatment is considered due and must be paid by you.

A service charge of 1 ½% per month (18%per annum) on the unpaid balance will be charged on all accounts exceeding 60 days, unless previously written financial arrangements are made. If the account goes to a collection agency, the full collection fee will be charged to the patient.  Additional fees will be applied for returned checks.

Any fee estimate for dental care can only be extended for a period of six months from the date of the patient examination.

Payment plans and financial arrangements are available for comprehensive dental treatment.  Please speak to us to make financial arrangements prior to commencing treatment.

Cancellation Policy

Appointment times are reserved especially for you. If you come in late, the Doctor may request that you re-schedule the appointment and you may be charged a missed appointment fee. If for any reason you should need to change your appointment please provide us a 2 business day notice in order to avoid a fee. A missed/cancelled hygiene appointment will have a cancellation fee of $125 and a surgery appointment will have a cancellation fee of $500. Please help us serve you better by keeping your scheduled appointments.
